Illness Reporting for Faculty, Staff & Students

To help monitor the H1N1 pandemic flu, the campus been asked to report faculty, staff and student absences due to flu or flu-like illnesses.

STUDENTS: If you are out sick, please:

  1. CLICK HERE to complete the online illness report.
     
  2. Contact your instructors to let them know you are sick or caring for others, and work with each instructor to make arrangements for missed classes and exams. If you need additional assistance, contact Student Services at (608) 355-5230.

FACULTY & STAFF: If you are out sick, please:

  1. CLICK HERE to complete the online illness report.
     
  2. Call the campus at (608) 355.5200, and let us know if you will not be coming in.
